Questions about FAST form

I printed out he FAST Fingerprint Pass and she took it with her to teh L1 office for printing. They signed it and gave it back to her.

Question 1: Does this need to be submitted with the rest of her paperwork? It is never mentioned on the CHL checklist.

Question 2: If it needs to be submitted, where do I get the "CHL Application ID#" from?

What the heck form is this that you guys are taking to L1?? I didn't get or see anything when I applied online and registered for the fingerprints.

I don't see anything on my checklist for any form to bring with you to the appointment that has to then be submitted with your packet to DPS...my understanding is that they submit the fingerprints for you. Perhaps you are referring to the CHL-6 form (photo holder/ID signature)?

Are you referring to the checklist itself with the barcode??

I'm doing my prints the monday after next and keep reading about a form to take to FAST and I have no idea what that is?? If there is a form - how do I get to it and print it out. When I log back into the DPS site, I don't see an option to print anything other than the checklist again.

The FAST form is on the L1 Enrollment website. You should have printed it when you made your appointment for prints. If you don't have it when you get to your appointment, you will be asked to fill in and sign a blank form.

You do NOT need to send it to DPS. You should keep it because it has your fingerprint trace number on it. If anything happens, and your fingerprint record does not get to DPS, it can be tracked with that number.
