No kidding on both of those. We've looked at replacing our current truck that is 13y old and coming up on 200k miles. Proper usage is this : things either happen "BY ACCIDENT" or "ON PURPOSE".
Some new words/usage make perfect sense such as "websurfing" or "couchsurfing",
but this "On accident" use seems clunky, awkward, and just plain wrong.
