If the firing pin spring was weak, it could fire by gravity, although unlikely. If the FP had been lubricated (bad idea), the heat of firing could lock it it the forward position, leading to slam-fire. Also unlikely. Maintenance matters.

However, proper training (gun handling) would have kept this from ever being in the news.

I have encountered quite a few decent shooters who were not as anal as I am about gun handling rules.

Another reminder that, as any LEO will tell you, "nothing good happens after midnight."
