If you can stand to wade through all the youtube comments, it appears the owner sent it back to Century, which diagnosed incorrect headspacing.
I've watched it numerous times and can't tell for sure, but it looks like it is possible that when the top cover blew off, it ricocheted off of the lens of his sunglasses/eye protection. Certainly there was some gas hitting him in the face, to pop that cover off.
It's also interesting to note the distinct lag between the gun blowing up and him reacting. You can see quite clearly in the slow motion part that it blows, pieces fly everywhere, the top cover is up as far as it will fly, and he is still in exactly the same position. There's a boom, a beat, and THEN he begins to move. Interesting demonstration of reaction time to an unexpected event.

I wouldn't give .02c for any AK out there. I heard them things cracking over in Vietnam and I don't need to hear that sound again.

Head space guages are cheap. I check every gun I build, even though I've been told it's not necessary. It only takes one out of spec part to wreck your day.
