Doing hand aerobatics and using what I remember of how I gripped the weapon here is what I believe happened:

My thumb came to rest on the very rear of the beaver tail behind the hammer, I can estimate how I gripped the weapon because of the angle of the minor cut the slide left on my thumb. My index finger was on top of the ejection port (This I can confirm clearly from memory). Based on where my Thumb and index fingers where located the swell of my palm was on the thumb safety (weapon has an ambi-safety). The swell of my palm must have disengaged the thumb safety (Watch the swell of your palm as you move your thumb) as I attempted to grip the weapon. My ring finger or my middle finger must have engaged the trigger while my little finger came to rest on the front strap.
