I have. A buddy of mine told me about a woman that was in his class. She was missing the target (not just the silhouette, THE WHOLE TARGET) at 3 yards. I've since seen this woman shoot, and to say that she was flinching and anticipating the recoil is an understatement. I saw her shoot at an intro to handguns course, and she was afraid of the recoil on a 22 revolver. There was no way she was going to pass the shooting portion without some serious one on one with an instructor. Most other people should have no problems passing.

I took my class from an LEO that has been teaching the class for quite some time. I can't recall the date he mentioned, but it's been at least several years. He mentioned that NO ONE has ever failed the shooting test. He did say he had to eject a few people though due to unsafe practices even after repeated warnings. This was for things like pointing the gun all over the place.

I witnessed a failure to pass, a renewal, and it was heartbreaking. The shooter had a pretty bad case of palsy; don't if it was Parkinson's or another condition, but the muzzle of his pistol swang uncontrollably in arcs of 3 or 4 inches. Now, I'll say up-front that I've seen people with similar palsy learn to shoot relatively well. They learned to understand the movement of their hands, and to anticipate the return of the muzzle to a certain point in order to fire when they were on target.
In this case, the gentleman either couldn't control/anticipate his condition well enough, or had not practiced enough to do so. The instructor gave him the maximum number of attempts to pass, and the shooter tried gamely each attempt. No go.
What made it so sad was that this was a renewal, and that the shooter was a WWII War veteran. I spoke to him briefly afterward. He no longer drove, but otherwise still got around mostly on his own. His grandson had driven him to the class, and the grandson was taking his first CHL class at the same. Neddless to say, the veteran was disappointed he was able to pass the shooting test.

We had one that fired one too many rounds at the wrong time and had to repeat, but passed with flying colors the second time. The one who actually failed the shooting portion was an older lady who was a husband of a former LEO.
I just got home from the CHL class - I passed!! I scored 249 in the practical and 98% on the written.
While it was a very long day (8am until almost 10pm) it was very informative. It was quite an eventful day. The instructor that was scheduled to take the class had a motorcycle accident on Saturday afternoon and was injured to the extent that he was admitted to hospital. Fortunately a replacement was found and despite the last minute circumstance and not being in his usual location did an admirable job.
Unfortunately that wasn't the only surprise of the day. After some instruction the class was divided in to two groups for the practical test. I was in the first group. There where several in the class who clearly had little or no experience with a handgun and really should not have been there, in my opinion. Frankly I was a little concerned regarding safety as there had only been a brief run through of range safety rules. That said I was taught to shoot by the RAF and the British military truly believes that learning can only be achieved by endless repetition.
After my group finished shooting we left the range area and everyone was adding up and comparing their scores while the next group were shooting. As we were doing this a member of the second group rushed out of the range area calling for someone to call an ambulance. For a moment everyone feared the worst, especially considering the skill levels we'd just seen. It soon became apparent that one of the older members of the class had fainted at the firing line. While he did fall he was not badly injured and remained conscious. EMS arrived very quickly and took him to hospital where we later learned he was admitted for observation but doing fine.
After that everything went OK although behind schedule. All said it was a fun, if exhausting, day. I wish I hadn't dropped that one shot into the "7" zone, but as there were only 2 perfect scores I felt I did OK as I was using a pistol I haven't yet owned for a week!
