Shucks, the knuckle dragger I am went about it a whole 'nother way.
I took the form, opened it with Foxit Phantom, and made a PDF form out of it. I put text boxes and check boxes where they are needed and then saved it. Piece of cake. If you enter your Instructor information and save the form under another name then you don't have to put all that in each time. You can fill out the student's information and save it. This only works if you open the form in Foxit or Acrobat. Acrobat reader will let you fill the form but, you can't save the information.
