If you can afford the entry fee, the Emerson LeGriffe is a good choice for last ditch defense. The blade cuts way out of proportion to its size. And if she's in the medical field, she probably knows what muscles and tendons to cut so it's anatomically impossible for him to grab her or chase her.

I like fixed blade knives for self defense. Waved knives usually work better than flippers or other opening devices on folders.
I agree with your points about ease of deployment, and secure grip/retention. Small & lightweight will make it easy for her to carry it.

Granted it is set up on the belt, but you could easily run it inside the waistband with a static cord.

Cold Steel has a model called the braveheart that may work. They also have the mini tac line, and Barry Dawson's Spike.
Boker has a few Fred Perrin design models. The la Griffe($65) and the neck bowie ($75) look the most promising.

If your wife likes the delica that she has then a similiar and more potent knife is a spyderco endura wave. It is a little larger, has a wave deployment system and is large enough to use as a weapon.
It is also small enough to not scare co-workers if discovered and is very similiar to the delica she already has and is comfortable with.

A few other carry options for her enviornment are a taser c2, pepper spray or other knives (spyderco miitary or police or she could get any cold steel product.)
