I've never had this happen, nor heard of it happening before this.
It was my first time.
I shot M16 in the service back in the 70's and never had anything like that happen. i wished I would have know I would have looked for the spent case. I would have loved to see what happened to it but I guess that what I get for buying bulk ammo.

Military ammo has staked primer pockets, so it's not likely to happen.

Hmm, wonder if the offending shell was a reloaded milsurp/milspec shell? Someone get a bit too enthusiastic in swaging out the primer pocket crimp?

I imagine you're right. I can't put my fingers on the article, but I do remember the warning about some less-than-reputable reloaders who tried to reuse military brass and had this problem.

I had some ammo that had a 100% primer ejection rate. Silver State Armory 77 OTM. I had those red lower receiver protectors and a primer still got under the rear of the trigger and would not let it reset, just like yours did. Also got in the opening in the upper where the cam lets the bolt rotate... that was fun to get out.
After 10 / 10 rounds tossing their primer, and the final straw was the little three point star primer piece getting behind the cam and in to the carrier, I gave up on that ammo. Must have been a bad lot. I still have it, might turn it.

I would and tell them it's junk. I would have never guess that was the problem. I shot it today with same ammo and all was good. i started with M855 62 Gr and sighted the scope in then switched to 55 gr and the lower target and wow nothing was center so I guess 62 gr is going to be my ammo for center mass. I looked around for the other failed case but could not find it, on a good side I got to shoot a 50 cal today just one shot but it was fun.
