P-38 Forum. That's where I saw some postings about this issue some months ago.
I posted a request for a link to the topic today on that forum, so hopefully will get some direction to find it back.
Follow up -- got a response.

Maybe it "can't happen"

to today's modern weapons, but .... apparently has happened to a few people with older semi-autos.

Regarding double action revolvers:
1. NYPD did a 10 year study some time ago...over 6000 officer involved shootings with the double action revolver and not a single malfunction was documented. (fact)
2. A quality action job will do wonders for the "shootability" of a double action revolver. (fact)
3. Most CHL'ers will not get the training required to be safe, effective and reliable with a pistol. (opinion)

Any of the above. "Mall Ninja" is an attitude, not limited to any particular job description.

Ahh, so a mall ninja is like what we called a 'woo-woo' in the ambulance service, or a 'black glover' when I worked corrections: guys who have sirens and lights on their own vehicles, or wear black leather gloves in uniform because 'it makes me intimidating'...
