tr 100 which copy

on the tr-100 i keep the yellow copy, and mail in the white copy right?

Mail in the original(white). The instructor get the pink and the yellow. You keep the goldenrod.

So, mail in the first page, middle two go to the instructor, bottom one is yours.

Make sure you and your instructor both signed it. And make sure that your name is spelled correctly, and that your TDL number is correct.

I give the top two to the student. That way I just tear them off and keep the attached bottom two.

Never seen any requirement to do otherwise?

on the sheet where it has your jobs residences etc.on the part about being in a chemically dependent facility, do i just leave all that blank or write a big N/A on it?

If you have been doing that successfully, then I am all for it. Now that I think about it, DPS can't possibly know what color the student got to keep, and that would be a lot easier. I just remember that, in my original instructor class, it was stressed that DPS got the top, student got the bottom, and the instructor(s) got the middle. I have been doing it that way ever since and never even thought about it.

I always says it's best to draw a diagonal line across any area where you have nothing to declare. That way, they know you intended to leave it blank and didn't just miss it.
