Is it possible for a round to go off while loading a mag?

I was in the Army and we used to load our M16 magazines then slap the back of the mag to make the rounds come all the way the back of the mag. I don't know how well thought out that was or if it even worked but while loading a magazine today and trying really hard to get the last few rounds in it occurred to me that it might be possible for one to go off while I was loading it. Does this ever happen?

It'd also be possible if the firing pin was somehow stuck forward, out the breech face.

It is unlikely however with ammo using an extremely soft primer that was not seated all the way with a piece of debris in the magazine that has a point on it being in just the right place, at just the right angle, at just the right time it could happen. Of course the round would also be likely to fire when the bolt/slide strips if from the magazine.

I would think it HIGHLY UNLIKELY. Never heard of it happening. I too picked up the habit of slapping the back end of my magazines against the heal of my hand. I can't remember where. I've just always done it. It may have been when I was in the Navy. There I was trained on M-16 and M-14.
