Why We Always Wear Eye Protection!

I was on the range with a student a few weeks ago. Student was shooting a revolver which either had a slightly loose cylinder or was slightly out of time.
The gun went bang and I felt something like a needle sticking me in the forehead. I pretty much knew what it was right away, and sure enough, a little bit of the bullet had shaved itself off entering the forcing cone and flown backwards and to the side where I was standing. If it had hit a little bit lower, and if I had not had on my eyepro, it would have stuck in my eye instead of my skin.

What make of revolver was this? I have heard of some Charter Arms doing this, because the cylinder doesn't precisely line up with the barrel.

It was an older S&W. When I looked at it, the cylinder didn't seem particularly loose or wiggly, but maybe it wasn't timed just right, or it was just barely loose enough to shave a little off of the bullet.
I told the student to take it to a gunsmith to be checked out for safety's sake.

My friend was a State champion pistol shot. She was practicing before a match when a fellow competitor shot a rock, the recochet knocked my friends cap off.
