I accidentally issued the previous LTC-100 form and students are receiving a letter asking for a current form. Am I to date it with the actual class date or is DPS wanting "today's" date ?

I would think the date of the class.

It's just a re-issue. Date should be actual completion date. Remember it has to match LTC-8 you already submitted
