Finger Print card

I went and got me fingerings taken but all the officer did was sign his name. There are a few blanks that I am nto sure about and was wondering when I need to place in these blanks.

DPS NO (SID) top center of card
POB top right (Is that place of birth "USA"?)

DPS NO (SID) top center of card <- leave blank
POB top right (Is that place of birth "USA"?) <- city and state of birth (they sure leave enough room, huh?)
