Address and Employment Field is Blank OK

Question on the address and employment field for my current address and employer. Since I'm sill at the current address for both should I leave the "to" field empty or use the current date?

Neither.

Say March 2005 (for example) to present.

If you use the date you fill out the form, that would imply that you left that job on that date.

Same principle applies to address.
