I would just ask the Doc if he has actually diagnosed you with anything.

For doctors to cover their behinds, and good practice, your medication and you diagnosis should always match. It would be be tough for a doctor to explain why there's a presciption for depression, anxiety, etc. if the person had never been diagosed with depression, anxiety, etc.

Still I imagine this would come up for a person only after receiving a CHL and something going horribily wrong that would cause the law to dig into this sort of info.

It is very common for doctors to prescribe "off-label".

Again, I think that only serious mental conditions are grounds for barring a person from having a CHL. Unless the condition is debilitating, I wouldn't worry. After all, who hasn't been depressed or anxious at some time in their life? IANAL nor an MD.
