Safariland 745BL Belt Clip question

Does anyone have one of these? I got my 5189-77 holster with the 745BL belt clip today. The instructions mention an "insert" that allows you to adjust the clip to three different sizes of belt. I think the clip was left out of the shipment. It was supposed to be "inside of the instructions", but there was nothing inside the instructions. There was an allen wrench taped to the back, which is used to adjust cant and tighten or loosen retention, but there was no insert.

I wish I could provide pictures, but this computer doesn't have any graphics editing capability.
