help....attach additional sheets?

Ok so I took my class.... and passed now im tring to get all the info in the packet filled out and find I need to add more work histoy then the paper allows. The bottom of the form states add additional sheets but does not give me a form number to add.
Im thinking I just need to add a chl-78a form from dps website. is there anything I would need to put on the form to indacate that this is an additional form?
any help would be great. thank you.

Before they had those fancy-dancy downloadable forms, "additional sheets" meant just plain old paper.

I think you could still do that, as long as you include all the info - dates, addresses, etc.
