I don't understand why is that checklist isn't clear at all , and clear for everyone?
It is asking for Fingerprints cards. I don't have a fingerprints cards, I had my fingerprints taken today, and they only gave me a receive.
Also every website is asking for passport photos , you don't need one if you let them to use your driver license photo.

If you born out of USA but you are a citizen now , what will you need?
Only the copies of the passport's front and back pages?

yes, all you need is a copy of your US Passport. Just a copy of the main page with your name and photo, etc. for the "photos", you just have to hand write on the CHL-6 form- "Please use my DL photo" (any open space on the form is fine)
