Probably the easiest thing is to apply with your Texas address and DL. Then if you decide you're really moving to Oklahoma, go ahead and get the Oklahoma DL and upload a scan of your new DL to the CHL site with your application barcode number. Or mail a photocopy of the new DL and tell them you moved for work. They're processing slow enough that you should have time.

Your CHL-100 is good for 2 years. It doesn't matter what DL you had at the time of your class. It doesn't matter if you've changed names between then & now. it doesn't matter if you've gained or lost 100 lbs. Point blank: you do not need to take a new course & you do not need a revised CHL-100 form.
