Mailing CHL checklist?

Which form's to send in and to write my application number on? I know the app it's self as well as the picture and fingure print's along with my arrest/court document's but am I missing anything? A check list would be awesome . If this has been covered already I appologize but I want to make sure I have all of t's crossed and i's dotted.
