Hi,
In my CHL Packet, there is a page where you are supposed to put your photo

On the Top Right it says

Application/License Number
(Then some boxes to write that down)

Last Name
(Then some boxes to write that down)

First Name, Middle Inital
(Then some boxes to write that down)

Date of Birth
(Then some boxes to write that down)

Texas ID
(Then some boxes to write that down)


Only problem is I dont know my application number, where is it? How do I figure that out.
Also the form tells me to use a Felt tip pen. what?

Mark, the application number is the number assigned to you IF AND ONLY IF you applied on line through DPS website. If you didn't apply on line then use your D.L.# But to be absolutely sure, before you go marking up the application, please let someone else verify that. I applied on line and my application # was in the application packet I received from DPS.

As far as the felt tip pen goes, I have no clue. I didn't use one. :shock:

You have an older photo card holder. It has recently changed, but if they sent it in the package, you can still use it.

If you got the photo card holder by applying online, then you have an application number. Look at the page that has all your info on it. In the top right hand corner is an eight digit number ending in "L" The number is your application number. Leave off the "L". That just designates that it is an online application.

If you just requested a package, but did NOT fill out an online application, then you will NOT have an application number and you can leave that section blank.

Fill it out with a BLACK felt tip pen, or a really good black ink pen. They will reject anything else. Make sure you AND your fingerprinter both sign the back of the photo before you stick it in the holder and tape it down. Sign your normal signature on the line under the photo.

Better yet, if your CHL class is a one-stop shop, don't fill out any paperwork until you get there. Let your instructor guide you through it.

Good grief, I can't believe how timely this information is. After sitting on my app for six months, I finally got fingerprinted and am trying to assemble the package today. These are the same questions I have. Only the government can make something so complicated. We should have a lot of fun when they take over health care.
